Paper Abstract:      Ageing has significant impacts on the organization of healthcare services, especially for elderly people affected by cognitive impairment. Recent findings suggest that physical exercise, a controlled diet, and intellectual and social stimulation are modifiable factors to prevent and slow down cognitive decline. Moreover, having information on the disease, creating strategies to minimize cognitive and functional disabilities, managing behavioral disturbances and reducing psychological distress can reduce the patients’ and caregiver burden. This paper defines a new service and organizational model enabled by a digital solution that meets these needs, and describes the related application within the DECI initiative, a Horizon 2020 project focused on elderly patients affected by Mild Cognitive Impairment and Mild Dementia. The paper describes the application of the model in two countries (Italy and Spain), which have similar characteristics, and reports the main lessons learnt, giving some advices on how successfully implement the model in other similar healthcare systems.
